2nd comments field - sign off

2nd comments field requested by CE as a way to communicate to facilitators / admin around the workshop.  For example, Brennan has read evaluation summary, wants to highlight an issue to facilitators.  This comments field is then attached to the evaluation summary so facilitators can see it when the admin sends the summary through after sign off.
